Bare Face RVA is a physician-led clinic specializing in the medical use of Botox for chronic migraine, neurological conditions, TMJ-related pain, and functional spasticity. Our goal is to expand access to evidence-based neurotoxin treatment in a boutique medical setting, without the long wait times or administrative barriers common in tertiary systems.


Conditions We Treat

We currently accept referrals for:

Chronic Migraine

• FDA-approved injection protocol
• Patients must meet diagnostic criteria
• Prior authorization required for most insurers

TMJ / Bruxism / Clenching

• Functional cases with pain, clicking, masseter hypertrophy, bruxism, or headaches
• Can reduce clenching forces and improve symptoms
• May be eligible for HSA/FSA

Neurological Spasticity

• Post-stroke spasticity
• Focal dystonia
• Lower facial dystonia
• Upper extremity functional impairment
• Post-TBI spasticity

Aesthetic Botox (Optional)

We offer natural cosmetic results under physician care for patients who request it — separate from medical indications.

We also accept self-referrals for migraine and TMJ.

Treatment is delivered by Sherita N. Chapman, MD, a dual board-certified neurologist and Associate Professor with nearly a decade of experience using Botox for chronic migraine and neurological conditions.

Insurance & Access Pathways

Botox may be covered for eligible medical indications. We support:

✔ Commercial insurance
✔ Medicare (in progress)
✔ Prior authorization
✔ HSA/FSA for eligible conditions
✔ Payment plans for non-covered care

If insurance coverage is denied or not available, patients may continue care through our Medical Access Membership, which ensures ongoing treatment at the correct dosing interval.

What to Include (If Available)

To expedite authorization and reduce administrative burden, the following documents are helpful:

For Migraine:
• Diagnosis + duration
• Headache frequency documentation
• Prior medication trials
• Imaging (if applicable)
• Recent clinic notes

For TMJ/Bruxism:
• Symptoms + duration
• Failed bite guards/splints
• Bruxism documentation
• Dental exam findings

For Spasticity:
• Diagnosis + etiology (ex: post-stroke)
• Goal of therapy (ex: gait, ROM, pain)
• Neuro/PT/OT notes
• Relevant imaging

Send what you have — referrals are not delayed for missing information.


Communication & Follow-Up

After evaluation, referring clinicians receive:

✔ Treatment summary
✔ Dosing information
✔ Response to therapy
✔ Functional outcomes (if applicable)

We coordinate with PT/OT when appropriate to maximize functional benefit.


Urgent Referrals

If a patient requires time-sensitive treatment (ex: severe spasticity compromising therapy), please indicate on your referral and we will prioritize scheduling.
