Medical consultations are used to evaluate candidacy for treatment, determine dosing, and initiate prior authorization when applicable.
Migraine Waitlist (Insurance-Based Care)
If you are seeking Botox for chronic migraine and are waiting on insurance credentialing or prior authorization availability:
We will contact you to complete intake, verify insurance, and schedule once credentialing is finalized.
This ensures you do not lose your place in line.
Medical Botox Inquiries
For neurological indications including:
• chronic migraine
• cervical dystonia
• spasticity
• TMJ with functional impairment
• hemifacial spasm
• other neurological conditions
This allows us to determine whether insurance coverage, prior authorization, or membership is appropriate.
Cosmetic Botox
For natural, physician-led cosmetic treatment:
Cosmetic patients do not need a consultation fee and may book directly.
Insurance & Billing Questions
We currently support:

✔ Medicare (in progress)
✔ Prior authorization for qualified indications
✔ HSA/FSA for eligible medical conditions
✔ Payment plans for non-covered care
✔ Cosmetic maintenance membership
If insurance coverage is not available, we offer a Medical Access Membership to ensure patients can still receive treatment.
