Appointment & Cancellation Policies
Bare Face RVA provides physician-led Botox treatment for chronic migraine, select neurological conditions, and natural aesthetic care. These policies protect appointment availability, support insurance coordination, and maintain timely access for all patients.
Cosmetic Botox Appointments
Booking Deposit
A $50 booking deposit is required to reserve a cosmetic Botox appointment. The deposit is applied to the cost of treatment at the visit.
If the patient elects not to receive treatment at the appointment, the deposit may be refunded or applied to a rescheduled visit.
Cancellation & No-Show
A $50 cancellation fee applies to any cosmetic appointment canceled or rescheduled with less than 24 hours’ notice, or not attended.
The forfeited deposit serves as the cancellation fee; no additional fee is billed.
Insurance-Based Medical Appointments
Insurance-based Botox appointments for chronic migraine and neurological conditions do not require a deposit.
Consultation & Billing
The initial consultation is billed to insurance and is used to:
confirm clinical candidacy
document diagnosis and treatment history
initiate prior authorization for Botox treatment
Standard copays, coinsurance, or deductibles may apply depending on the patient’s plan.
Insurance Verification Prior to Consultation
Insurance benefits for the consultation are verified prior to the appointment. If the plan does not cover the consultation, patients will be notified 48–72 hours prior to the visit to review alternatives.
Prior Authorization for Botox Treatment
Botox injection appointments are not scheduled until prior authorization has been completed and approved.
Botox medication is not billed until after authorization approval. Patients are not responsible for the cost of Botox medication unless their insurance denies coverage following prior authorization.
Cancellations & No-Show for Insurance Appointments
A $50 cancellation fee applies to insurance-based appointments canceled or rescheduled with less than 24 hours’ notice, or not attended.
The cancellation fee for insurance appointments is billed through the patient portal.
Late Arrival
Patients arriving more than 10 minutes past their scheduled time may need to be rescheduled to allow adequate time for care. Late arrivals may be treated as late cancellations when appropriate.
Payment Methods
Bare Face RVA accepts credit/debit cards, HSA, and FSA payments for eligible services. Cosmetic Botox is not covered by insurance.
